Стаття присвячена актуальним питанням інтенсифікації функціонування системи охорони здоров’я на
рівні закладів охорони здоров’я із застосуванням системи економічних інструментів та методів. Досліджено
можливості формування та впровадження організаційно-економічного механізму управління закладом охорони здоров’я для забезпечення його розвитку. Проаналізовано та систематизовано підходи до створення
організаційно-економічного механізму управління закладом охорони здоров’я. Окреслено сукупність соціальних, економічних та організаційних важелів, які діють на заклад охорони здоров’я з метою зростання його
конкурентоспроможності, ефективності функціонування та якості надання медичних послуг. Надано пропозиції щодо побудови організаційно-економічного механізму управління закладом охорони здоров’я в контексті
пацієнт-орієнтованого підходу для забезпечення його розвитку.
The article is devoted to current issues of intensifying the functioning of the healthcare system at the level of
healthcare institutions using a system of economic tools and methods. The purpose of the article is to study the
possibility of forming and implementing the organizational and economic mechanism for healthcare management
in the context of a patient-oriented approach to ensure its development. The article analyzes and systematizes
approaches to creating the organizational and economic mechanism for managing the healthcare institution.
The basic model and key elements for the formation of the basic architecture of the organizational and economic
development mechanism for the healthcare institution have been formed. The most complex problematic issues that need to be addressed by the management of healthcare institutions in modern conditions of reform are outlined.
Proposals are given for building the organizational and economic mechanism for managing the healthcare institution
in the context of the patient-oriented approach to ensure its development. The functioning of two subsystems of the
mechanism was studied: the functional business system and the support and development system. It is emphasized
that the functional subsystem is subject to mandatory state licensing, as well as the implementation of norms
and standards of state regulation (unified clinical protocols, local clinical protocols, etc.). The organizational and
economic mechanism for the development of the healthcare institution is considered as a set of the legal system,
functional system of business processes, of support and provision business processes, of business development
processes. The main problems and negative factors of introducing the new organizational and economic mechanism
for managing the healthcare institution are considered. Additional attention is paid to such the important element of
the modern organizational and economic mechanism of healthcare management as the introduction of internal audit
of activities into the medical and management system of the healthcare institution. A set of social, economic and
organizational levers acting on healthcare is outlined in order to increase its competitiveness, operational efficiency
and quality of medical services. It has been determined that the proposed organizational and economic mechanism
for healthcare management is a set of approaches, means, techniques, methods and tools, indicators and norms
and other elements that can have the complex impact on the results of healthcare activities and ensure the increase
in the efficiency of its functioning.
