Використання різних рентгенологічних методів дослідження в діагностиці верхньощелепного синуситу

Abstract

В статті представлені результати частоти використання різних рентгенологічних методів діагностики хронічного одонтогенного гаймориту. У результаті проведеного дослідженні встановлено, що найбільш часто використовуваними методами рентгенологічного обстеження при хронічних одонтогенних верхньощелепних синуситах є ортопантомографія (ОПТГ) та оглядова рентгенографія в підборіддя-носовий проекції (ОР). Висока частота використання ОПТГ і ОР в діагностиці хронічних одонтогенних гайморитів обумовлено наступними обставинами: 1) значна частина обстежених первинно зверталися до оториноларингологів, які призначали ОР з метою вивчення змін гайморових пазух; 2) широке використання ОПТГ при діагностиці хронічних одонтогенних гайморитів, очевидно, пов'язано з необхідністю диференціації одонтогенного і риногенного гайморитів. Дані літератури не дозволяють робити висновок про інформативність методів, що найчастіше використовуються рентгендіагностиці хронічного одонтогенного гаймориту. У зв'язку з чим, питання інформативності цих методів залишається невирішеним.

The odontogenic infection becomes the reason of the development of maxillary sinusites in 25-30%. The odontogenic sources of infection are periapical nidi of inflammation, juxtahilar cysts, foreign bodies. The grounds of the diagnostics of sinusites are the clinical examination and the data of roentgenologic investigation. The Aim of the Investigation. The study of the recommendations for the use of some methods of roentgenologic investigation at the different forms of chronic odontogenic maxillary sinusitis. The Materials and Methods. We have observed 45 in-patients with chronic odontogenic maxillary sinusitis: 14 (31.1 %) men and 31 (68.9 %) women at the age from 24 to 60 years, undergoing treatment in the Department of Maxillo-Facial Surgery and Dental Surgery of the UE MCHU and FA in Zaporizhia during 2011-2012. The random selection of the patients took place. The clinical diagnosis was made after the thorough examination according to the local protocols of the diagnostics and treatment of odontogenic maxillary sinusitis. The findings and the discussion of them. The orthopantomography (OPTG) and the plan radiography in mental-nasal projection (PR) are the most frequently used methods of roentgenologic investigations at chronic odontogenic maxillary sinusitis and were held in 100% of cases. This high frequency of the use of OPTG and PR at the diagnostics of chronic odontogenic maxillary sinusites is conditioned by the following circumstances: the considerable part of the examined patients at first attended otolaryngologist, who prescribed PR for the study of the changes in maxillary sinus; the wide use of OPTG at the diagnostics of chronic odontogenic maxillary sinusitis is, obviously, connected to the necessity of differentiation of odontogenic and rhinogenous maxillary sinusites. The literary data do not allow drawing a conclusion on the informativity of the most frequently used methods of roentgenologic diagnostics of chronic odontogenic maxillary sinusites.
