Клинико-лабораторное обоснование эффективности разработанного комплексного метода профилактики и лечения осложнений химиотерапии в полости рта у больных раком молочной железы

Abstract

Полученные результаты свидетельствуют о том, что для эффективного лечения осложнений химиотерапии в полости рта у онкологических больных необходим дифференцированный, патогенетически обоснованный, индивидуальный подход к выбору схемы лечения и используемых препаратов, благодаря мониторингу генетической предрасположенности к оральному мукозиту перед введением химиотерапии, а также динамики стоматологического здоровья пациента в процессе лечения.

Отримані результати свідчать про те, що для ефективного лікування ускладнень хіміотерапії в порожнині рота у онкологічних хворих необхідний диференційований, патогенетично обґрунтований, індивідуальний підхід до вибору схеми лікування і використовуваних препаратів, завдяки моніторингу генетичної схильності до орального мукозиту перед введенням хіміотерапії, а також динаміки стоматологічного здоров'я пацієнта в процесі лікування.

Our previous studies have shown the high therapeutic efficacy of the developed supporting complex for the prevention and treatment of complications of chemotherapy in the oral cavity of patients with breast cancer (namatsit, kaltsid, laktovit, lizomukoid, kvertulin). However it is necessary to identify certain patterns of this disease and to suggest the outcome of treatment, which will give the opportunity to build an adequate treatment plan and determine the frequency of application, thereby increasing the clinical effectiveness of the developed scheme. The aim is to identify the individual genetic characteristics of breast cancer patients and to assess the risk of chemotherapyinduced oral mucositis, and to follow the dynamics of changes in the status of the oral mucosa and periodontal tissues in patients receiving supporting therapy complex. Materials and methods. This study involved 39 patients with breast cancer at the age of 29-67 years ( 22 - core group , 17 – comparison group) . The allelic variants of genes COL1A1, GSTM1, IL-6, TNF, p53, CALCR, ER-α were stutied. We studied the condition of the periodontal tissues, oral mucosa, oral hygiene, electrical performance of buccal cells in the dynamics of the treatment. Results. Conclusions. The results indicate the fact that for the effective treatment of complications of chemotherapy in the oral cavity of cancer patients it is necessary to make a differentiated, pathogenesis reasonable, individual approach to the choice of treatment and the drugs used. This is possible by monitoring the genetic susceptibility to oral mucositis before the administration of chemotherapy, as well as the dynamics of the dental health of the patient during treatment. We have developed the complex which provides stable remission and stabilization of the pathological process.
