Частота ускладнень із боку верхньощелепного синусу при хронічному періодонтиті зубів верхньої щелепи

Abstract

Хронічний періодонтит на сьогодняшній день залишається основною причиною видалення зубів, окрім того, хронічний періодонтит зубів верхньої щелепи здатний призвести до розвитку ускладнень у вигляді хронічного одонтогенного гаймориту. Частота захворювань періодонта в даний час не має тенденції до зниження. При лікуванні хронічного періодонтиту спостерігається великий відсоток невдач як у найближчі, так і у віддалені терміни. Потреба у переліковуванні кореневих каналів зубів перевищує потребу в їх первинному лікуванні.

Treatment of patients with chronic periodontitis is one of the most difficult and important tasks of modern medicine. Nowadays chronic periodontitis remains the leading cause of tooth extraction, furthermore, chronic periodontitis of the teeth of the upper jaw can lead to the extention of complications such as chronic odontogenic sinusitis. Currently the frequency of periodontal disease has no tendency to decrease. In the treatment of chronic periodontitis there is a large percentage of failures, both in the immediate and distant in time. The need for retreatment of root canals overcomes the need for their initial treatment. Mechanism of odontogenic sinusitis is usually associated with sensitization of the maxillary sinus mucosal microflora odontogenic focus. The appearance of focus of inflammation in the periapical tissues of the upper molars is accompanied by destruction of bone tissue. This causes the thinning of the layer of bone that separates the elite from the roots of teeth maxillary sinus. This fact, together with the anatomical features of the structure (intimate location of the tops of the roots of the teeth and the sinus floor) is the distribution of causes of the inflammation of the mucous membrane of the maxillary sinus. In the structure of sinus mucosa marked hemodynamic disturbances are revealed in the form of sludge erythrocytes and thrombosis, regional standing neutrophilic leukocytes. Infected foreign bodies (feeling materials) getting to the sinus cavities cause the development of chronic inflammation with marked proliferation of the mucosa. Difficulties in the diagnosis of maxillary sinusitis are connected with the fact that the process is primary chronic. Therefore, the increased attention of physicians to detect this disease will significantly decrease the severity of complications. The aim of the study is to determine the frequency of complications of chronic periodontitis of the teeth as chronic odontogenic sinusitis.

Хронический периодонтит на сегодняшний день остается основной причиной удаления зубов, кроме того, хронический периодонтит зубов верхней челюсти способен привести к развитию осложнений в виде хронического одонтогенного гайморита. Частота заболеваний периодонта в настоящее время не имеет тенденции к снижению. При лечении хронического периодонтита наблюдается большой процент неудач как в ближайшие, так и в отдаленные сроки. Потребность в перелечивании корневых каналов зубов превышает потребность в их первичном лечении.
