The article presents a review-based analytical synthesis of modern approaches to optimizing rehabilitation after total hip arthroplasty during the acute and post-acute stages. The relevance of the topic is determined by the fact that surgical elimination of pain and restoration of anatomical support do not automatically ensure completefunctional recovery, because postoperative mobility depends on muscle reserve, postural control, fall risk, cognitive ability to follow instructions, adherence to exercise, comorbidity profile and access to rehabilitation support. The aim of the study wasto compare enhanced recovery pathways, early mobilization, weight bearing as tolerated, home-based exercise programs, supervised physical therapy, telerehabilitation, virtual reality, progressive resistance training, balance and gait training, aquatic exercise and activity liberalization in terms of effectiveness, safety, clinical indications and further development prospects. The subject of this study is approaches to the rehabilitation of patients following joint replacement surgery. The study was designed as a narrative review with elements of a systematized search of publications from 2021 to 2026 in PubMed, Scopus, Web of Science, Cochrane Library and Google Scholar. Hip replacement surgery helps eliminate chronic pain, restore weight-bearing function to the limb, and significantly improve the quality of life for patients with end-stage degenerative, post-traumatic, or necrotic hip joint conditions. The analysis showed that ERAS is the most justified framework for the acute stage, as it creates physiological and organizational prerequisites for early mobilization through multimodal analgesia, complication prevention, patient education and coordinated multidisciplinary care. During the post-acute stage, a structured home program is appropriate for low-risk patients with adequate self-management, telerehabilitation is preferable when regular feedback and adherence support are required, whereas supervised physical therapy is clinically prioritized in patients with gait instability, abductor weakness, fear offalling, cognitive barriers, poor technique or delayed functional progress. The review emphasizes that virtual reality, aquatic exercise, balance training and progressive resistance training should not be interpreted as interchangeable technologies, because each intervention targets a different mechanism of recovery and has a specific indication profile. The most promising direction is a personalized hybrid rehabilitation pathway that combines pre-discharge risk stratification, a minimum set of functional outcomes, remote monitoring, selective face-to-face supervision and technology-assisted modules matched to the patient’s dominant functional deficit. Optimizing rehabilitation after hip replacement should be based on a phased approach and risk stratification. In the acute phase, the most evidence-based approach is the ERAS protocol with early mobilization. It creates the physiological and organizational conditions for safe movement, rather than simply increasing the number of exercises.
