Objective. To identify and systematize dermoscopic criteria for the assessment of the arrangement and distribution of cutaneous vascular patterns, taking into account pathophysiological changes in inflammatory dermatoses and skin neoplasms, with the aim of improving the accuracy of differential and non-invasive diagnosis.
Materials and Methods. A narrative review and systematic synthesis of scientific publications, consensus recommendations, and
clinical studies in dermoscopy were conducted, focusing on the spatial organization of cutaneous vascular structures – specifically
their arrangement and distribution – in inflammatory dermatoses and skin neoplasms.
Results and Discussion. Contemporary literature indicates that the spatial characteristics of vascular patterns – arrangement and
distribution – serve as informative dermoscopic indicators of microcirculatory and angiogenic alterations in the skin and reflect the
key pathophysiological mechanisms underlying inflammatory dermatoses and skin neoplasms. Regular and uniform patterns of vascular arrangement and distribution are more commonly associated with reactive and inflammatory processes as well as certain benign lesions, whereas focal, clustered, radially oriented, or arborizing patterns combined with central or irregular distribution demonstrate higher nosological specificity for particular types of neoplasms. At the same time, no single pattern of vascular arrangement or
distribution is absolutely specific and therefore requires comprehensive interpretation in conjunction with vascular morphology, background dermoscopic structures, and the clinical context.
Conclusions. Standardized assessment of the arrangement and distribution of dermoscopic vascular features improves interpretative reproducibility, enhances the diagnostic value of dermoscopy, and facilitates more accurate differential evaluation of inflammatory and neoplastic skin lesions.
