Оклюзійні зміни, що виникають після встановлення
ортопедичних конструкцій з опорою на дентальні
імплантати змінюють баланс у роботі протилежних
жувальних та скроневих м’язів. Ціль нашої роботи.
Вивчити нейром’язову реакцію жувальних м’язів
(Musculus Masseter, Tempralis Anterior) на встановлення незнімних ортопедичних конструкцій з опорою
на дентальні імплантати для лікування часткової
вторинної адентії в динаміці (3 місяці). Матеріали
та методи дослідження. Нами було обстежено
15 пацієнтів (11 жінок і 4 чоловіків) в віці від 30 до
55 років. У всіх було виготовлено незнімні ортопедичні конструкції з опорою на дентальні імплантати
в бокових ділянках щелеп (у 11 на нижній щелепі і 4 на
верхній щелепі). Дефекти були однобічні. Обстеження
проводились на час здачі ортопедичної конструкції
з опорою на дентальні імплантати та через 3 місяці
після. Активність м’язів Musculus Masseter, Tempralis
Anterior реєстрували за допомогою бездротової ЕМГ
системи Teethan®. Вивчали наступні показники: РОС,
BAR, ASIM, TORS, ІMP. Результати дослідження.
Коефіцієнт РОС жувальних та скроневих м’язів,
індекс BAR, індекс ASIM, індекс TORS індекс ІMP на
час встановлення ортопедичної конструкції вказували на відсутність в синхронізації роботи досліджувальних м’язів, що притаманно для однобічного
жування. Через 3 місяці ці показники наближались до
діапазону норми. Висновки. Після фіксації незнімної
ортопедичної конструкції з опорою на детальні імплантати, що заміщує однобічні дефекти зубних рядів
в бокових ділянках щелеп, у пацієнтів зберігається
стереотип жування який проявляється у вигляді дисбалансу активності роботи пари жувальних та передніх скроневих м’язів, що притаманно для однобічного жування. Показники індексів: POC, BAR, ASIM,
TOPS, IMP значно відрізняються від діапазону норми
(р˂0,001). Через 3 місяці, після заміщення однобічного дефекту зубного ряду незнімною ортопедичною конструкцією з опорою на дентальні імплантати, активність пари передніх скроневих та жувальних м’язів
наближаються до збалансованої контлатеральної
активності.
Occlusive changes that occur after the installation of
orthopedic structures based on dental implants change
the balance in the work of the opposite masticatory
and temporal muscles. Purpose of our work. To study
the neuromuscular response of masticatory muscles
(Musculus Masseter, Tempralis Anterior) to the
installation of non-removable orthopedic constructs based
on dental implants for the treatment of partial secondary
adentia over time (3 months). Materials and methods of
research. We examined 15 patients (11 women and 4 men)
aged 30 to 55 years. All were made of non-removable
orthopedic structures with support on dental implants in
the lateral parts of the jaws (11 on the lower jaw and 4 on
the upper jaw). The defects were one-sided. Examinations
were conducted at the time of delivery of the orthopedic
structure based on dental implants and 3 months after.
Musculus Masseter, Tempralis Anterior muscle activity
was recorded using the Teethan® wireless EMG system.
The following indicators were studied: ROS, BAR, ASIM,
TORS, IMP. Results of the study. The ROC coefficient of
the masticatory and temporal muscles, the BAR index, the
ASIM index, the TORS index, the IMP index at the time
of establishing the orthopedic structure indicated the
absence of the study muscles in synchronization, which
is inherent in unilateral chewing. After 3 months, these
indicators approached the normal range. Conclusions.
After fixing the non-removable orthopedic structure with
support on detailed implants, replacing the unilateral
defects of the dentitions in the lateral parts of the jaws,
patients retain the stereotype of chewing, which manifests
itself in the form of an imbalance in the activity of a pair of
chewing and anterior temporal muscles, which is inherent in unilateral chewing. Index indicators: POC, BAR, ASIM,
TOPS, IMP significantly differ from the normal range (p
˂ 0.001). After 3 months, after replacing the unilateral
defect of the dentition with a non-removable orthopedic
structure based on dental implants, the activity of a pair of
anterior temporal and masticatory muscles approaches a
balanced contralateral activity.
