Сучасні моделі кластерних лікарень та досвід Ізраїлю, Швеції і Туреччини для імплементації в Україні та одеському госпітальному окрузі

Abstract

Сучасні системи охорони здоров’я функціонують в умовах зростаючої складності, що зумовлює необхідність пошуку ефективних моделей організації медичної допомоги. У статті досліджено міжнародний досвід кластеризації лікарень на прикладі Ізраїлю, Швеції та Туреччини. На основі порівняльного аналізу обґрунтовано підходи до формування гібридної моделі кластеризації лікарень для умов України. Використано методи порівняльного аналізу, системного підходу та узагальнення. Визначено ключові характеристики різних моделей кластерів та доведено доцільність їх інтеграції. Запропонована модель враховує організаційні, економічні та психологічні чинники функціонування системи охорони здоров’я. Практичне значення полягає у можливості її застосування в межах госпітальних округів України.

The increasing complexity of modern healthcare systems, driven by demographic shifts, digital transformation, resource constraints, and crisis conditions, necessitates the development of more effective organizational models for healthcare delivery. Traditional hierarchical hospital management structures are increasingly unable to ensure efficiency, coordination, and resilience. In this context, hospital clusterization is considered a promising approach to integrating healthcare institutions into coherent functional systems. This study provides a comparative analysis of hospital clusterization models in Israel, Sweden, and Turkey and substantiates approaches to developing a conceptual hybrid model adapted to the Ukrainian healthcare system. The research employs comparative analysis, a systemic approach, and conceptual modeling. The findings identify three core models of hospital clusters: vertically integrated, territorially coordinated, and infrastructure-based hub models. Each model demonstrates specific strengths and limitations depending on institutional context, governance structure, and resource distribution. The results indicate that none of these models can be directly transferred without adaptation to transitional economies. The scientific contribution lies in the development of a hybrid clusterization model that integrates organizational, economic, and behavioral dimensions, including leadership, inter organizational trust, and resistance to change. Unlike existing approaches, the proposed model emphasizes multi-level integration and a balance between centralization and decentralization. The study is limited by its conceptual nature and lack of empirical validation. The practical significance of the research consists in providing a conceptual framework for transforming hospital districts into integrated healthcare clusters in Ukraine, particularly within regional systems. The proposed model can be used as a strategic tool for improving healthcare management efficiency, coordination of medical services, and overall system resilience under conditions of ongoing reforms and external challenges.
