Когнітивні порушення (КП) – один з багаточисленних проявів
розсіяного склерозу (РС). Попри те, що даний дефіцит зустрічається у 40–70%
пацієнтів з РС та негативно впливає на повсякденну активність, поведінку
та якість життя у цілому, йому надається значно менше уваги, ніж руховим
проявам. Незважаючи на соціально-економічні труднощі, які завдають пацієнтам
саме КП, їх внесок у загальний бал за шкалою EDSS мінімальний, а тому
залишається непоміченим багатьма клініцистами, а діагностика даного дефіциту є досить випробувальною, оскільки потребує кваліфікованих спеціалістів
у сфері когнітивних розладів та достатньо часу для повноцінного тестування.
Мета роботи – систематизувати дані про клінічні та патогенетичні особливості
КП у пацієнтів з РС, а також можливі маркери прогнозу даних порушень.
Матеріали та методи. Проведено компʼютеризований пошук у базах даних
PubMed та Web of Science за ключовими словами «когнітивні порушення»,
«розсіяний склероз», «МРТ» та «біомаркери» за 2020–2025 рр.
Результати. На динаміку КП впливають безліч факторів – від структурних змін
до функціональних, як от порушення сну та тривожно-депресивні розлади.
Все більшої уваги приділяється біомаркерам у крові та спинномозковій рідині,
які можуть виступати предикторами когнітивних розладів і дозволяють прогнозувати їх перебіг. КП прогресують часто незалежно від активності основного процесу та кількості загострень і можуть навіть бути першим симптомом РС. Їх наявність у пацієнтів з клінічно ізольованим синдромом прогнозує не тільки більш
виражене погіршення когнітивної функції, а й загальну агресивність процесу.
Втім, не існує загальноприйнятого рішення щодо методів діагностування даних
порушень, як і їх попередження і лікування.
Висновки. Незважаючи на наявність безлічі досліджень у контексті КП при РС,
не існує загальноприйнятого рішення щодо причини, методів виявлення
та корекції даних симптомів. Є потреба у подальших дослідженнях у контексті
саме КП, їх скринінг на ранніх етапах РС та приділення більшої уваги даному
аспекту захворювання, що надасть змогу попередити або призупинити
прогресування когнітивної дисфункції, а отже і покращити стан пацієнтів
та їх соціальну адаптацію у майбутньому.
Background. Cognitive impairment (CI) is one of the numerous manifestations of
multiple sclerosis (MS). Despite the fact that this deficit occurs in 40–70% of patients
with MS and negatively affects daily activities, behavior, and overall quality of life, it receives significantly less attention than motor symptoms. Although CI brings
considerable socio-economic difficulties to patients, its contribution to the overall
EDSS score is minimal, and therefore often remains unnoticed by many clinicians.
Moreover, diagnosing this deficit is quite challenging, as it requires qualified specialists in the field of cognitive disorders and sufficient time for comprehensive testing.
Purpose – to systematize data on the clinical and pathogenetic features of CI in patients with MS, as well as possible prognostic markers of these impairments.
Materials and Methods. A computerized search was conducted in PubMed and
Web of Science databases using the keywords «cognitive impairment», «multiple
sclerosis», «MRI», and «biomarkers» for the years 2020–2025.
Results. The dynamics of CI are influenced by numerous factors – from structural
to functional, such as sleep disorders and anxiety-depressive disorders. Increasing
attention is being paid to blood and cerebrospinal fluid biomarkers, which may
serve as predictors of CI and allow forecasting of their progression. CI often progresses
independently of the activity of the underlying disease and the number of relapses
and may even represent the first symptom of MS. Their presence in patients with
clinically isolated syndrome predicts not only more pronounced cognitive decline
but also overall aggressiveness of the process. However, there is still no universally accepted approach to the methods of diagnosing, preventing, or treating
these impairments.
Conclusions. Despite the abundance of studies on CI in the context of MS, there is
no generally accepted solution regarding the causes, diagnostic approaches, and
correction of these symptoms. There is a need for further research focusing specifically on CI, their screening at the early stages of MS, and greater attention to this
aspect of the disease, which could allow prevention or slowing of cognitive decline
progression and will improve patientsʼ condition and their future social adaption.
