Регіональні та професійні особливості інтенсивності карієсу зубів у дорослого населення деяких регіонів України

Abstract

Карієс зубів у дорослих залишається однією з провідних стоматологічних проблем, що визначає рівень працездатності, якість життя та потребує значних ресурсів системи охорони здоров’я. Особливої уваги потребують особи зрілого та похилого віку, для яких характерні виражені зміни структури твердих тканин зубів, накопичення стоматологічної патології та вплив різних виробничих і екологічних чинників. Незважаючи на значний обсяг даних щодо дитячого та молодого населення, стоматологічний статус осіб віком понад п’ятдесят п’ять років вивчений недостатньо, що зумовлює актуальність даного дослідження. Мета дослідження – визначити інтенсивність ураження твердих тканин зубів і структуру індексу КПВ серед дорослого населення деяких регіональних і професійних груп України. Матеріали і методи. Обстежено 131 особу віком 55-69 років з міст Одеси, Білої Церкви, Черкас, Кривого Рогу, Дніпра та смт Машівки. Дослідження проводилося у виробничих колективах з використанням стандартного клінічного огляду відповідно до ре-комендацій ВООЗ. Визначали індекс КПВ та випадки ускладненого карієсу. Отримані дані подано у вигляді середнього значення та стандартної похибки. Результати. Найнижчі показники ураження твердих тканин зубів виявлені серед мешканців ендемічного щодо флюорозу району Машівки, тоді як найвищі значення зафіксовано у гірників з вібраційною хворобою та працівників металургійного заводу. У структурі показника у всіх групах переважали пломбовані та видалені зуби, тоді як частка активного карієсу поступово зменшувалася з віком. У робітників хімічного виробництва відзначено співвідношення між каріозними та пломбованими зубами, характерне для тривалої дії хімічних чинників. В офісних працівників Кривого Рогу виявлено помірні значення стоматологічної захворюваності, тоді як у гірників спостерігалося значне збільшення числа видалених зубів. Загалом дані свідчать про різкий вплив умов праці на стоматологічний статус осіб зрілого та похилого віку. Висновки. Інтенсивність ураження твердих тканин зубів у дорослих суттєво залежить від регіону проживання та професійних умов. Встановлено переважання видалених і пломбованих зубів у структурі індексу в усіх групах, що відображає накопичений характер патології в зрілому та похилому віці. Результати підкреслюють необхідність розробки цільових профілактичних програм для окремих професійних груп та регіонів.

Dental caries in adults remains one of the leading oral health problems, affecting work capacity, quality of life, and requiring substantial healthcare resources. Individuals of mature and elderly age represent a particularly vulnerable group due to pronounced changes in the structure of dental hard tissues, accumulation of dental pathology, and long-term exposure to various occupational and environmental factors. Despite the considerable amount of data available for children and young adults, the dental status of individuals over fifty-five years of age has been insufficiently studied, which determines the relevance of the present research.The aim of the study was to determine the intensity of carious lesions of dental hard tissues and the struc-ture of the decayed, missing, and filled teeth index among adult populations belonging to several regional and occupational groups of Ukraine.Participants and methods. A total of one hundred and thirty-one individuals aged fifty-five to sixty-nine years were examined in the cities of Odesa, Bila Tserkva, Cherkasy, Kryvyi Rih, Dnipro, and in the settlement of Mashivka. The study was conducted in organized industrial collectives using standard clinical examination procedures in accordance with the recommendations of the World Health Organization. The decayed, missing, and filled teeth index and cases of complicated dental caries were assessed. The obtained data were presented as mean values with standard error.Results. The lowest levels of dental hard tissue damage were observed among residents of the fluorosis-endemic area of Mashivka, whereas the highest levels were recorded in miners with vibration disease and workers of the metallurgical plant. In all examined groups, filled and missing teeth predominated in the overall index structure, while the proportion of active caries decreased progressively with age. Workers of the chemical industry demonstrated a characteristic ratio of carious to filled teeth associated with long-term exposure to chemical factors. Office workers of Kryvyi Rih exhibited moderate levels of dental morbidity, whereas miners had a marked increase in the number of missing teeth. Overall, the data indicate a strong influence of occupational conditions on the dental status of mature and elderly individuals.Conclusions. The intensity of damage to dental hard tissues in adults is significantly influenced by region of residence and occupational factors. A predominance of missing and filled teeth was observed in all groups, reflecting the cumulative nature of dental pathology in mature and elderly age. The findings highlight the need for targeted preventive programmes for specific professional and regional populations.
