Infection control in the conditions of a multidisciplinary hospital

Abstract

The purpose of the study is to evaluate the effectiveness of the system of infection control measures in the conditions of a multidisciplinary hospital. The study was performed on the basis of the multidisciplinary medical center (university clinic) of the Odesa National Medical University in 2023. The results of epidemiological monitoring were analyzed (the number of postoperative infectious complications, the length of stay of patients in the hospital). In addition, an analysis of the qualitative composition of microbiocenoses of exudates obtained from drains in the early postoperative period and biofilms fixed on drains and catheters was performed. In addition, 20 employees of departments of a multidisciplinary medical center were interviewed regarding the importance of various risk factors for SSI and other types of postoperative infectious complications. Statistical processing was carried out by methods of variance analysis using Excel software (MS Office 2021, Microsoft Inc., USA). In 62.5% of cases, confirmed SSI were registered in patients of surgical departments. The frequency of SSI in gynecological patients did not exceed 9.2% of their total number in the hospital. The studied samples are dominated by multicomponent microbial associations, consisting mainly of Str. haemolyticus, Str. epidermidis, Str. viridans, Str. Warneri, St. capitus, St. aureus, Ent. faecium, E.coli. The type of surgical intervention, its duration and the state of microbiocenoses of the patient’s skin are the main factors in the success of infection control measures in a multidisciplinary hospital.

Метою дослідження є оцінка ефективності системи заходів інфекційного контролю в умовах багатопрофільного стаціонару. Дослідження виконане на базі багатопрофільного медичного центру (університетська клініка) Одеського національного медичного університету у 2023 р. Проаналізовані результати епідеміологічного моніторингу (кількість післяопераційних інфекційних ускладнень, тривалість перебування хворих в стаціонарі). Додатково проведений аналіз якісного складу мікробіоценозів ексудатів, одержаних по дренажах у ранньому післяопераційному періоді, та біоплівок, фіксованих на дренажах і катетерах. Додатково опитано 20 працівників відділень багатопрофільного медичного центру щодо важливості різних чинників ризику ІМХВ та інших видів післяопераційних інфекційних ускладнень. Статистична обробка проводилася методами дисперсійного аналізу за допомогою програмного забезпечення Excel (MS Office 2021, Microsoft Inc., США) У 62,5% випадків підтверджені ІМХВ реєструвалися у хворих хірургічних відділень. Частота ІМХВ у гінекологічних пацієнтів не перевищувала 9,2% від загального їх числа у стаціонарі. У досліджених зразках переважають мультикомпонентні мікробні асоціації, що місять переважно Str. haemoliticus, Str. epidermiditis, Str. viridans, Str. warneri, St. capitus, St. aureus, Ent. faecium, E.colі. Вид оперативного втручання, його тривалість та стан мікробіоценозів шкіри пацієнта є основними факторами успішності заходів інфекційного контролю у багатопрофільному стаціонарі.
