Оцінка імунного статусу пацієнтів із хронічним панкреатитом у віковому аспекті

Abstract

Мета роботи: вивчити та проаналізувати стан імунної системи (ІС) пацієнтів із хронічним панкреатитом (ХП) у віковому аспекті. Матеріали і методи. Дослідили показники ІС 161 пацієнтів на ХП, яких було розподілено в залежності від віку на три групи: до 45 років (54 хворих), від 46 до 65 років (76 обстежених), старше 65 років (31 пацієнт). Контрольну групу склали 25 практично здорових осіб. Вивчали кількість загальної популяції Т-лімфоцитів, субпопуляцій Т-хелперів і Т-супресорів які визначали у цитотоксичному тесті з використанням моноклональних антитіл імуноферментним методом за рівнем експресії мембранних антигенів. Оцінку структурного стану підшлункової залози (ПЗ) у хворих на ХП проводили, підсумовуючи дані УЗД для визначення ступеня важкості процесу. Оцінку екскреторної недостатності ПЗ (ЕкН ПЗ) визначали за рівнем фекальної α-еластази (ФαЕ) методом імуноферментного аналізу. Результати. Визначено зміни Т-клітинної ланки імунітету за рахунок вірогідного зниження у пацієнтів молодого віку рівня показників CD3, CD4, CD8, CD16 та CD 72 на 44,8; 36,1; 24,4; 32,2 та 18,4 %% відповідно (р<0,001); в середньому віці імунна недостатність поглиблювалась, що проявлялось зниженням зазначених показників на 54,6; 37,2; 29,9; 41,4 та 25,6 %% відповідно (р<0,001); у пацієнтів віком понад 65 років визначено вірогідну Т-лімфоцитопенію за означеними показниками на 66,4; 47,8; 37,7; 70,8; 41,5 %% (р<0,001) у порівнянні із контрольною групою. Із збільшенням віку хворих на ХП спостерігалась неспецифічна активація гуморального імунітету за рахунок збільшення показників IgG, IgА,IgМ у хворих із ХП молодого віку на 6,7; 13,3; 30,1 %% (р<0,001); на 9,6; 25,1; 30,1 %% в середньому віці (р<0,001) та на 16,9; 30,9; 41,4 %% у віці понад 65 років (р<0,001) відповідно у порівнянні із здоровими обстеженими. Виявлені тенденції щодо зниження активності комплементу на 56,6 % у молодих пацієнтів на 54,8 % в середньому та на 78,2 % у похилому віці (р<0,001) і збільшення ЦІК у контингенті підтвердилися і поглиблювалися з постарінням хворих на 58,9; 75,8 та 69,1 %% відповідно (р<0,001). Висновки. На основі регресійно-кореляційного аналізу доведено, що вік хворих, тривалість ХП, рівень фекальної α-еластази, структурний стан ПЗ за критеріями УЗД є достовірними предикторами формування імунодефіциту при ХП.

Purpose. To study and analyze the state of the immune system (IS) of patients with chronic pancreatitis (CP) in the age aspect. Materials and methods. We studied the IS parameters of 161 patients with CP, with an average age of (58.17±2.46) years, which were divided depending on the biological age into three groups: up to 45 years (54 patients), from 46 to 65 years (76 examined ), older than 65 years (31 patients). The control group consisted of 25 practically healthy individuals. The following parameters of immunity were studied: the number of the total population of T-lymphocytes (CD3), B-cells (CD72), and subpopulations of T-helpers / inducers (CD4) and Tsuppressors / killers (CD8), natural killers (CD16), which determined in a cytotoxic test using monoclonal antibodies of classes CD3, CD4, CD8, CD16, CD72 by enzyme immunoassay by the level of expression of membrane antigens. The assessment of the structural state of the pancreas (PG) in patients with CP was performed by summing up the ultrasound data to determine the severity of the process. Assessment of excretory insufficiency of the pancreas (ECI of the pancreas) was determined by the level of fecal α-elastase (FE) by enzyme immunoassay. Results. Changes in the T-cell link of immunity were revealed due to a probable decrease in the level of CD3, CD4, CD8, CD16 and CD 72 in young patients by 44.8; 36.1; 24.4; 32.2 and 18.4%, respectively (p<0,001); in middle age, immune deficiency worsened, which was manifested by a decrease in these indicators by 54.6; 37.2; 29.9; 41.4 and 25.6%, respectively (p<0,001); in patients older than 65 years, probable T-lymphocytopenia was determined according to the indicated indicators by 66.4; 47.8; 37.7; 70.8; 41.5% (p<0,001) compared to the control group. With an increase in the age of CP patients, nonspecific activation of humoral immunity was observed due to an increase in IgG, IgA, IgM in young patients with CP by 6.7; 13.3; 30.1% (p<0.001); by 9.6; 25.1; 30.1% in middle age (p<0,001) and by 16.9; 30.9; 41.4% over 65 years of age (p<0,001), respectively, compared with healthy subjects. The revealed tendencies for a decrease in complement activity by 56.6% in young patients by 54.8% on average and by 78.2% in the elderly (p<0,001) and an increase in the CIC in the contingent were confirmed and deepened with aging of patients by 58.9; 75.8 and 69.1%, respectively (p<0,001). Conclusions. Based on regression-correlation analysis, it was proved that the age of patients, the duration of CP, the level of fecal α-elastase, the structural state of the pancreas according to ultrasound criteria are reliable predictors of the formation of immunodeficiency in CP.
