Метою дослідження було з’ясування основних патофізіологічних механізмів дегенеративно-дистрофічного&#13;
ураження суглобів з метою визначення лікувальних заходів, спрямованих на покращення функціонування суглобів та&#13;
відновлення повноцінного живлення внутрішньосуглобної хрящової тканини. У цьому огляді досліджено сучасні&#13;
етіологічні чинники та патофізіологічні механізми дегенеративно-дистрофічних та запальних уражень великих суглобів&#13;
нижніх кінцівок з точки зору обґрунтування доцільності застосування лікувальної тактики пролотерапії гіпертонічними&#13;
розчинами глюкози та/або декстрози. Систематичний аналіз досліджень з 2020 по 2025 роки, взятих з медичних баз даних&#13;
PubMed, Google Scholar, Web of Science and SCOPUS засвідчує складність та каскадність патофізіологічних механізмів&#13;
остеоартрозу, що суттєво ускладнює клінічний перебіг захворювання і робить малоефективною медикаментозну терапію.&#13;
Автори акцентують увагу на пролотерапії – методиці регенеративної медицини, застосування якої шляхом введення в&#13;
пошкоджені ділянки колінного суглобу області гіпертонічні розчини глюкози та/або декстрози забезпечує усунення&#13;
хронічного болю та стимулює природне відновлення внутрішньосуглобних тканин. Клінічні дані підтверджують&#13;
ефективність пролотерапії у зниженні болю та покращенні рухливості, забезпечуючи кращі результати порівняно з&#13;
традиційними методами лікування. Автори висловлюють альтернативну важливість пролотерапії для хворих на гонартроз,&#13;
які шукають неінвазивні варіанти лікування захворювання.; The purpose of the study was to provide a comprehensive overview of the pathophysiological mechanisms underlying&#13;
degenerative-dystrophic joint damage and to identify therapeutic approaches to improve joint function and intra-articular cartilage&#13;
nutrition. This review examines the current etiological factors and pathophysiological mechanisms of degenerative-dystrophic and&#13;
inflammatory lesions of the large joints of the lower limbs, with a view to explaining the rationale for the use of prolotherapy with&#13;
hypertonic glucose and/or dextrose solutions. A systematic analysis of studies from 2020 to 2025, drawn from the medical&#13;
databases PubMed, Google Scholar, Web of Science, and SCOPUS, demonstrates the complexity and cascade of osteoarthrosis&#13;
pathophysiological mechanisms, which significantly complicate the clinical manifestations of the disease and render drug therapy&#13;
ineffective. The authors focus on prolotherapy, a regenerative medicine technique that uses hypertonic glucose and/or dextrose&#13;
solutions, injected into damaged areas of the knee joint, to relieve chronic pain and stimulate the natural regeneration of intraarticular tissues. Clinical data confirms prolotherapy's efficacy in reducing pain and improving joint mobility, often achieving&#13;
better results than traditional treatments. In their conclusion, the authors emphasize the alternative importance of prolotherapy for&#13;
patients with gonarthrosis who are looking for non-invasive treatment options.

The aim. To determine the distribution features of various forms of Helicobacter pylori infection and&#13;
their concentration on the gastric mucosa in various topographic zones in patients with chronic&#13;
atrophic gastritis, suffering from chronic constipation.&#13;
Materials and methods. A comprehensive examination of 100 patients with chronic non-atrophic&#13;
gastritis was carried out, of which 50 patients suffered from chronic constipation (main group), and 50&#13;
patients did not suffer from constipation (control group), in which the presence of various forms was&#13;
determined, as well as the degree of colonization of Helicobacter pylori infection in various&#13;
topographic zones of the stomach.&#13;
Results. When analyzing the results, it was found that in the group of patients suffering from&#13;
chronic constipation, the frequency of occurrence of the active form of Helicobacter pylori infection in&#13;
different topographic zones of the stomach ranged from 46% to 56% with an average degree of&#13;
mucosal colonization of 1.12 ± 0.13 (+) up to 1.48 ± 0.13 (+), the frequency of occurrence of an inactive&#13;
form of Helicobacter pylori infection in different topographic zones of the stomach ranged from 0% to&#13;
48% with an average degree of mucosal colonization from 0 ± 0.0 (+) to 1.30 ± 0.13 (+), while while in&#13;
the group of patients who did not suffer from chronic constipation, the frequency of occurrence of a n&#13;
act distinct Helicobacter pylori infection in different topographic zones of the stomach ranged from&#13;
36% to 52% with an average degree of mucosal seeding from 1.02 ± 0.13 (+) to 1.34 ± 0.13 (+), and the&#13;
frequency of occurrence of the inactive Helicobacter pylori infections in different topographic zones of&#13;
the stomach ranged from 0% to 58% with an average degree of mucosal seeding from 0 ± 0.0 (+) to 1.56&#13;
± 0.13 (+).In patients of the first group, the frequency of detection of intracellular "depots" of HP&#13;
infection was 2.3 times higher than in patients of the second group.&#13;
Conclusions. In patients with chronic non-atrophic gastritis, suffering from chronic constipation, the&#13;
degree of contamination of the gastric mucosa with active forms of HP infection is significantly higher&#13;
in the body of the stomach by a large curvature than in patients with chronic non-atrophic gastritis who&#13;
do not suffer from constipation. The frequency of detection of intracellular "depots" of HP infection in&#13;
patients with chronic non-atrophic gastritis, suffering from chronic constipation, is 2.3 times higher&#13;
than in patients with chronic non-atrophic gastritis who do not suffer from constipation, which indicates a longer and more common pathological process the effect of HP infection on the&#13;
digestive process, in which the intestines are also involved.&#13;
All human studies were conducted in compliance with the rules of the Helsinki Declaration of the&#13;
World Medical Association "Ethical principles of medical research with human participation as an object&#13;
of study". Informed consent was obtained from all participants.
