<summary type="text">Molecular characteristics of endometrial cancer: entering the era of precision medicine

Endometrial cancer ranks third among cancers affecting women in Ukraine, with a&#13;
tendency to incidence rate increase every year. The disease is extremely rare in women under&#13;
40, and uncommon between 40 and 50, but thereafter, as age increases, the incidence curve&#13;
rises sharply, peaking at around 63 years of age. As a rule, the disease is diagnosed at an early&#13;
stage, with an overall 5-year survival rate exceeding 95%. However, this rate drops&#13;
significantly in cases of regional or distant spread, standing at 68% and 17% respectively.&#13;
Historically, endometrial carcinoma was classified into two main clinical-pathological types.&#13;
The first type comprised endometrioid adenocarcinomas, whilst the second included nonendometrioid tumours (serous, clear cell, undifferentiated carcinomas and carcinosarcomas).&#13;
However, the work carried out in compiling The Cancer Genome Atlas (TCGA) has&#13;
transformed our understanding of the molecular landscape of endometrial cancer, identifying not two but four molecular subtypes: tumours with mutations in the POLE gene,&#13;
microsatellite-unstable carcinomas, tumours with mutations in the TP53 gene, and carcinomas&#13;
of an unspecified molecular subtype. The integration of molecular classification into clinical&#13;
practice has altered the stratification of patients into risk groups, which, in turn, has&#13;
influenced the choice of adjuvant therapy. The rehabilitation of patients and ensuring a high&#13;
quality of life is a very complex task, and to address it, an appropriate balance is required&#13;
between reducing the risk of recurrence and preventing side effects associated with unjustified&#13;
escalation of treatment.&#13;
This review presents information on the molecular subtypes of endometrial cancer,&#13;
surrogate markers for each molecular subtype, and methods for their determination. The&#13;
prognostic significance of molecular classification is discussed, as well as the prospects for its&#13;
use in the design of future studies.

The study was aimed to determine the platinum-resistance biomarkers in patients with ovarian cancer. 29 platinum-resistant patients with verified serous ovarian cancer of IIIA–IIIC stages were examined, who received 6 courses of adjuvant chemotherapy with platinum preparations (cisplatin 75–100 mg/m2 intravenously with hydration and forced diuresis every 3 weeks) in the postoperative period. As control, 26 patients with non-recurrence of the disease performed for 6 months post-treatment observation (platinum-susceptible tumors) were observed. The expression of ABCA1 is higher in platinum-resistant cases of ovarian cancer. The most pronounced relationship exists between the values of survivin expression, local activity of nitric oxide, the expression of glutathione dependent enzymes, the content of catecholamines in erythrocytes, and the expression of ABCA1 transporter.

Objective. To   evaluate   the   clinical   and   morphological   features   of   malignant transformation  of  ovarian  endometrioma  and  to  emphasise  the  importance  of  oncological vigilance in the management of patients with endometriosis.Materials and methods.A clinical case of early endometrioid carcinoma of the ovary against  the  background  of  endometrioma  in  a  woman  of  reproductive  age  is  presented.  An analysis  of  current  literature  data  and  ESGO,  ESHRE,  and  JSGO  recommendations  for  the diagnosis and treatment of such patients wasperformed.Results.Morphological  and  radiological  signs  of  increased  oncological  risk  were identified,  which  became  the  basis  for  timely  surgical  intervention.  Endometrioid  carcinoma G1, FIGO IA was histologically verified. Fertility-preserving staging surgery was performed. No recurrence was detected during 24 months of follow-up.Conclusions.Ovarian  endometrioma  can  be  a  source  of  early  gynaecological  cancer processes  even  in  young  patients.  Vigilance,  high-quality  imaging  and  adequate  surgical tacticsare key factors for successful treatment.

The aim of this study was to assess the prevalence of HPV infection among women in different regions of Ukraine.Materials  and  methods.  An  analysis  of  data  from  the  Quintip  Recordati  database, containing 195 records, was performed using frequency analysis methods. The distribution of different  HPV  strains  among  the  women  surveyed,  who  live  in  different  regions,  was determined.  An  analysis  of  the  frequency  of  seropositive  results  depending  on  the  degree  of urbanisation was performed. The main ways of preventing HPVinfection were identified.Results. It was found that the number of positive tests accounted for 38.5% of the total sample.  When  comparing  the  frequency  of  detection  of  high-oncogenic  strains,  it  was  found that they are more often registered in cities with larger populations. Conclusions:1.  The  prevalence  of  HPV  in  the  women  examined  is  twice  the European average.2. The frequency of detection of highly oncogenic HPV strains is higher in urbanised  regions.3.  Widespread  implementation  of  HPV  screening  methods  in  Ukraine could significantly reduce the incidence of cervical cancer.

The purpose of the study was to evaluate the human papillomavirus infection prevalence among women in different&#13;
regions of Ukraine using the Quintip Recordati database. The Quintip Recordati database data, containing 195 records, was&#13;
analyzed using frequency analysis methods. The distribution features of different HPV strains among the surveyed women living&#13;
in other regions were determined. The frequency of seropositive results was analyzed also depending on the degree of urbanization.&#13;
It was found that the number of positive tests was 38.5 % of the total sample size. When comparing the frequency of detection of&#13;
highly oncogenic strains, it was found that they are more often registered in cities with a larger population. The authors showed&#13;
that the prevalence of human papillomavirus infection in the surveyed women is twice higher comparing the average European&#13;
level. Papillomavirus infection high prevalence in Ukraine is supposed to be the result of low vaccination coverage, lack of mass&#13;
screening, lack of awareness and limited access to preventive programs. The frequency of detection of highly oncogenic HPV&#13;
strains is higher in urbanized regions. The authors conclude that human papillomavirus infection diagnostic screening methods&#13;
widespread implementation in Ukraine can significantly reduce the incidence of cervical cancer. They are sure that cervical cancer&#13;
prevention will continue to rely on secondary prevention methods.; Метою даного дослідження була оцінка поширення папіломавірусної інфекції серед жінок різних регіонів&#13;
України. Проведений аналіз даних бази Quintip Recordati, що містить 195 записів методами частотного аналізу. Визначені&#13;
особливості розподілу різних штамів папіломавірусної інфекції серед обстежених жінок, які проживають у різних&#13;
регіонах. Проведений аналіз частоти серопозитивних результатів в залежності від ступеня урбанізації. Встановлено, що&#13;
кількість позитивних тестів склала 38.5 % від загальної чисельності вибірки. При порівнянні частоти виявлення&#13;
високонкогенних штамів встановлено, що вони частіше реєструються у містах з більшим населенням. Автори висвітлили,&#13;
що поширеність папіломавірусної інфекції в обстежених жінок вдвічі перевищує середньоєвропейські рівні. Скоріше за&#13;
все, висока поширеність папіломавірусної інфекції в Україні є наслідком низького охоплення вакцинацією, відсутності&#13;
масового скринінгу, браку обізнаності та обмеженого доступу до профілактичних програм. Частота виявлення&#13;
високоонкогенних штамів ВПЛ вище в урбанізованих регіонах. Автори висловлюють, що широке впровадження&#13;
скринінгових методів діагностики ВПЛ в Україні може суттєво зменшити захворюваність на рак шийки матки. Вони&#13;
впевнені, що профілактика раку шийки матки й надалі спиратиметься на методи вторинної профілактики.

The standard treatment for late stages remains cytoreductive surgery combined with systemic chemotherapy. However, survival and quality of life for patients with the traditional approach remain unsatisfactory.&#13;
Aim: The aim of the study was to analyse the quality of life of patients with primary advanced ovarian cancer depending&#13;
on the type of treatment tactics before the start of specialized treatment, after cytoreductive surgery in the early and late&#13;
postoperative periods, during chemotherapy and after the end of specialized treatment.

The aim of the work is to assess the quality of life in platinum-resistant patients with ovarian  cancer,  depending  on  the  degree  of  pharmacoresistance  and  differentiated  use  of chemotherapy.Materials  and  methods. The  study  was  conducted  on  the  basis  of  the  University Clinic of the Odessa National Medical University during 2014 -2020 years. 350 patients with adenocarcinoma  of  the  ovaries  of  III-IV  stage  were  examined,  who  performed  cytoreductive operations.Results  and  discussion. It  is  shown  that  the  initial  values  on  the  subscales  of  the questionnaires  EORTC  QLQ-C30  and  FACT-G  in  patients  classified  as  different  clinical groups were compared. When using a differentiated approach in the treatment of patients with OC,  the  indicators  on  the  scales  of  physical  (PF),  role  (RF)  and  emotional  functioning  (EF) were  significantly  improved.  In  addition, the  intensity  of  nausea  (NV)  and  general  weakness (FA)  decreased  in  patients  of  groups  III  and  IV.  In  the  case  of  differentiated  use  of chemotherapeutic  agents  with metabolic  support,  the  total  score  according  to  the  FACT-G questionnaire  was  77.80.9  points  in  group  III,  and  77.90.8  points  in  group  IV,  which significantly  exceeds  the  received  in  I  and  II  groups -72.21,2  and  71,60,9  points.  The described differences were kept throughout the period of catamnestic observation.Conclusions: The use of a differentiated approach in the treatment of patients with RI significantly  improved  indicators  on  the  scales  of  physical  (PF),  role  (RF)  and  emotional functioning  (EF).  After  treatment,  the  intensity  of  nausea  (NV)  and  general  weakness  (FA) decreased  in  patients  of  groups  III  and  IV.  The  described  differences  were  retained  for  12 months  after  the  completion  of  the  course  of  treatment.  Prospects  for  further  research  are related  to  the  study  of  the  dynamics  of  life  quality  of  patients  with  OC  at  the  subsequent stages of catamnestic observation.

<summary type="text">A case report of cytoreductive surgery of metastatic adrenal cancer in a patient with tumor progression

Background: Adrenocortical cancer is a rare malignancy with high risk of relapse and disease progression. The approaches to the treatment of relapse cases are not sufficiently clearly defined by current clinical guidelines.&#13;
Case Description: Male patient O., born in 1967, has been suffering from the cancer of right adrenal gland since 2020. The patient underwent right-sided epinephrectomy in April 2020. Later he got several courses of chemotherapy with the mixed results. Then he underwent laparoscopic cytoreductive assisted excision of the recurrent neoplasm of the right adrenal gland on 04 April 2024, and received everolimus 10 mg per day postoperatively. There was evidence of recurrence of adrenocortical cancer of the adrenal gland with invasion of the fibrous capsule without signs of tumor growth at the edges of the resection. According to immunohistochemistry data, there was a decrease in phosphatase and tensin homolog deleted on chromosome 10 (PTEN) expression, an increase in Ki-67 expression, and a lack of programmed death- ligand 1 (PD-L1) expression. According to multispiral computed tomography data from May 2024 they found an increase in cells in the lungs by 2 mm, an increase in cells in the liver by 18 and 6 mm, an increase in paracaval lymph nodes by 12 mm. The given case of relapse and progression of the disease is of interest, because the removal of part of the liver during cytoreductive surgery and preservation of the kidney improve the quality of life. Due to limited resources hyperthermic intraperitoneal chemotherapy (HIPEC) and other methods of intraoperative chemotherapy were not applied.&#13;
Conclusions: Cytoreductive surgery is an important option for complex treatment in the advanced stages of adrenocortical cancer.

<summary type="text">New conception of cancer prevention strategy in the Odesa region: modern program and personificated approach

The authors analyse the state of cancer care in Ukraine and its future trends, taking&#13;
into account the impact of the war. The paper highlights and analyses the current aspects of&#13;
the problem of early diagnosis of cancer against the background of reforming medicine in&#13;
general and the oncological service in particular. The main problems of the oncological&#13;
service in the region and the state of the organisation of diagnosis of malignant pathology by&#13;
family doctors are shown. The experience of solving the main problems of the lack of highquality oncological diagnostics, oncological prevention and interaction between primary and&#13;
other levels of oncological care at the level of Odesa is presented, and a modern approach to&#13;
improving the state of the oncological service in the region is proposed. The main vectors of&#13;
development of the oncological service in the framework of healthcare reform in Ukraine are&#13;
described.; Авторами проведений аналіз стану онкологічного сервісу в Україні та&#13;
його майбутніх тенденцій з урахуванням та впливом війни. В работі висвітлено та&#13;
проаналізовано сучасні аспекти проблеми ранньої діагностики онкологічних&#13;
захворювань на тлі реформування медицини в цілому та онкологічної служби зокрема.&#13;
Показано основні проблеми онкологічного сервісу регіону та стан організації&#13;
діагностики злоякісної патології сімейними лікарями. Наведено досвід вирішення на&#13;
рівні міста Одеса основних проблем відсутності якісної онкодіагностики, онкопревенції&#13;
та взаємодії між первинною та іншими ланками надання онкологічної допомоги, а&#13;
також запропонований сучасний підхід до покращення стану онколгічної служби&#13;
регіону. Описано основні вектори розвитку онкологічної служби в рамках&#13;
реформування медицини в Україні.
