105 patients, aged 18–40 y.o. with late HIV and active skin-mucousal symptoms have been examined. The number of CD4 Т-lymphocytes was not more than 200/mkl. In 94 patients (99%) the III stage of HIV development has been diagnosed, i.e. the stage of secondary diseases with severe chronic skin pathology. 95 endoscopic examinations of the gastrointestinal tract upper parts has been done for this patients. The analysis of the endoscopic data of HIV/TB patients showed that in 93 persons (98%) there was asymptomatic pathology of esophagus, stomach, duodenum. Esophagus secondary damages, according to the results of the endoscopic examinations, are met more frequently and include candidosis which is more typical for HIV late stages. Esophagus damages may be asymptomatic but there are skin and mucousal memebranes damages typical for the level of blood CD4 Т-lymphocytes < 200 kl/mm3 ( pneumocystosis, cryptococcosis, hispoplasmosis, fungous infections, etc.). So, the pathology of the gastrointestinal tract upper parts in late-infected HIV/TB-patients may be asymptomatic with accompanied skin pathology constitutes nearly 98%. Specifity of skin and mucousal damages, level of CD4 Т-lymphocytes < 200 kl/mm3 is an important sign for additional examination of the gastrointestinal tract upper parts. Endoscopic examination in the group under investigation is the only objective and most informative method.
Обследовано 105 больных в возрасте от 18 до 40 лет на поздних стадиях ВИЧ-инфекции с активными кожно-слизистыми проявлениями, у которых число CD4 Т-лимфоцитов не превышало 200/мкл. На основе анализа результатов эндоскопического исследования пищевода можно сделать вывод, что патология верхних отделов желудочно-кишечного тракта, которая может протекать бессимптомно на поздних стадиях у ВИЧ/ТБ-инфицированных больных с кожной патологией, составляет около 98 %. Специфика поражения кожи и слизистых оболочек, уровень CD4 Т-лимфоцитов < 200 кл/мм3 являются важным ориентиром для планового обследования верхних отделов желудочно-кишечного тракта. Эндоскопическое исследование у больных на поздних стадиях ВИЧ/ТБ с кожными проявлениями и отсутствием жалоб со стороны желудочно-кишечного тракта — единственный объективный метод, который при использовании всех дополнительных методик наиболее информативный.
