У статті здійснено системний клінічний аналіз структури,
причин і механізмів діагностичних помилок при різних формах хвороби Лайма у
дітей на підставі критичного огляду сучасних вітчизняних і міжнародних
наукових джерел. Наголошено, що хвороба Лайма є найпоширенішою
трансмісивною інфекцією Північної півкулі та набуває в Україні дедалі більшої
медичної й соціальної значущості через значне зростання кількості випадків
упродовж 2000–2023 рр.
Обґрунтовано, що збудник Borrelia burgdorferi sensu lato зумовлює
полісистемне ураження організму із вираженим клінічним поліморфізмом, у
зв’язку з чим хвороба Лайма розглядається як «великий імітатор». Виявлено три
основні категорії діагностичних помилок. Схарактеризовано проблему диференційної діагностики Лайм-артриту, який у дітей часто плутають із септичним
артритом та ювенільним ідіопатичним артритом. Наголошено, що нейропсихіатричні прояви нерідко інтерпретуються як первинні психічні розлади із
затримкою діагностування від кількох місяців до років.
Розкрито значення помилок серологічної діагностики, зокрема хибнонегативних результатів імуноферментного аналізу у ранні терміни інфікування,
хибнопозитивних реакцій, труднощів інтерпретації імуноблоту та використання
методів із низькою специфічністю.
Виявлено такі системні та організаційні чинники: недостатня пильність
лікарів у малоендемічних регіонах, обмежена доступність швидкої діагностики
та наявність расових диспаритетів. Підтверджено, що у значної частки дітей із безеритемними формами
відсутня вказівка на укус кліща, а рівень IgG суттєво вищий порівняно з ізольованою мігруючою еритемою. Доведено доцільність удосконалення діагностичних підходів шляхом створення проспективних реєстрів помилок, розроблення
адаптованих алгоритмів діагностики й упровадження цільових освітніх програм
для лікарів.
The article presents a systematic clinical analysis of the structure,
causes, and mechanisms of diagnostic errors in various forms of Lyme disease in
children, based on a critical review of contemporary national and international
scientific sources. It is emphasized that Lyme disease is the most common vector-borne
infection in the Northern Hemisphere and is gaining increasing medical and social
significance in Ukraine, driven by a significant increase in cases during 2000–2023.
It is substantiated that the causative agent, Borrelia burgdorferi sensu lato, leads
to multisystem involvement with pronounced clinical polymorphism, which is why
Lyme disease is regarded as the “great imitator.” Three main categories of diagnostic
errors have been identified.
The problem of differential diagnosis of Lyme arthritis is characterized, as it is
often confused in children with septic arthritis and juvenile idiopathic arthritis. It is
emphasized that neuropsychiatric manifestations are frequently interpreted as primary
mental disorders, resulting in delays in diagnosis ranging from several months to years. The significance of errors in serological diagnostics is highlighted, including
false-negative enzyme-linked immunosorbent assay results in the early stages of
infection, false-positive reactions, difficulties in interpreting immunoblot results, and
the use of methods with low specificity. Systemic and organizational factors have been
identified, including insufficient clinical alertness among physicians in low-endemic
regions, limited access to rapid diagnostics, and the presence of racial disparities.
It has been confirmed that a significant proportion of children with nonerythematous forms do not report a tick bite, and that IgG levels are significantly higher
than in isolated erythema migrans. It is concluded that there is a need to improve
diagnostic approaches by creating prospective registries of diagnostic errors,
developing adapted diagnostic algorithms, and implementing targeted educational
programs for physicians.
